Expert Reply:
One of the Draw-Tite hitches available for your vehicle won't allow full sized spare tires to remain under the vehicle, but this isn't the case with the etrailer # e98858 hitch.
As long as your 2008 Highlander is equipped with the 245/55-19 stock tire size, the etrailer brand Class III hitch you've looked at, part # e98858 will fit and will allow the 19 inch spare to remain in place under the vehicle once the hitch is installed.
If you plan on towing with the hitch, you'll also need a trailer wiring harness like part # 118449, a ball mount like # 2753 and for a 1-7/8 inch trailer ball, use # 19260 or # 63845 for a 2 inch ball.
I've linked to install videos for the hitch and the trailer wiring harness mentioned above.
