Will this hitch work with a 18 inch factory wheel with the measurement with the tire being 28.5 inches wide?

Yes, the Draw-Tite Max-Frame Trailer Hitch Receiver item # DT84MR will allow for an 18" full size spare tire to fit underneath your 2022 Toyota Highlander. I was able to check with Draw-Tite and confirm this information. The item # DT84MR will accept a full size spare tire but only on a 18" or 20" steel wheel, not an aluminum wheel. They did not specify the diameter of your tire only that if your vehicle has the factory spare you're good.
