I would be happy to help you find the correct bearing kit for your 2004 EZ Loader trailer, but unfortunately we do not have a break down of the specific bearings your trailer uses.
Instead you will need to remove one of your hubs and look for the bearing numbers stamped in the face of each bearing as shown in the included picture of the # L44649.
If your bearing are worn to the point that you cannot read the bearing numbers then you will need to use digital calipers # PTW80157 to precisely measure your spindles in the locations shown in the included diagram.
If you can provide the above information I would be happy to help you find the correct bearing kit for your trailer.
