Replacement Circuit Board Recommendation for MaxxFan Roof Vent Assembly 6401K

Question:

I have the fan pictured without any #'s on the fan cover. The fan motor part# is 10-04020 and a date of 06/2015. The fan sometimes starts and sometimes doesn't. When it does start it runs for various lengths of time and quits. The motor runs when the switch is by passed and the switch was tested in all run positions and has no continuity. Do you have circuit board for this fan.

Thanks for the picture! Based on it I can tell that the correct circuit board is the part # MA10-04301k-v16 that you referenced! The series of fan you have is the 6401k.
