Motor home receiver at 20, Blue Ox baseplate at 13.5inch above pavement, for 6.5inch difference. Better to use 4inch 2.5inch higher than baseplate or 6inch 0.5inch above baseplate?

Ideally your tow bar will be level when it is connected to both your motorhome and your vehicle's base plates. At most it can be 3-inches high or low on either end. Based on your dimensions the 6-1/2-inch height difference you have means you'll want to use a high/low hitch adapter that provides at least 6-inches of drop.
