Hi, is the only difference between this drum assembly and the EZ lube version the dust cap?

You are correct. The only difference between the standard spindle Trailer Hub and Drum Assembly - 3,500-lb Axles - 10" Diameter - 6 on 5-1/2 - Pre-Greased # AKHD-655-35-K and EZ-Lube spindle Easy Grease Trailer Hub and Drum Assembly for 3.5K Axles - 10" - 6 on 5-1/2 - Pre-Greased # AKHD-655-35-EZ-K is going to be the grease cap. Well, other than the price as well.
The standard one uses the Grease Cap - 1.988" Outer Diameter - Drive In # F001518 and the EZ-Lube one uses the 2" Lubed Dust Cap & Rubber Plug - 1.99" x 1.40" x 1.18" - Qty 1 # DC200L-DCRP (though the # AKHD-655-35-EZ-K actually includes both caps for some reason).
