The vent cover I recommend for your MaxxFan Plus Roof Vent # MA00-04500K is the MaxxAir FanMate # MA00-955002. I also recommend picking up the Bug Screen # MA00-955203 to help keep bugs from making a home under the FanMate.
This uses the EZClip system so the installation is a breeze (pun intended).
If you're just needing a replacement lid for the MaxxFan Plus then part # MA10A40009K is what you're looking for.
