We have what you need! I went ahead and contacted Dexter with your serial number for your axle, and you have a Dexter 5200 pound axle with EZ Lube spindles and 6 on 5-1/2 hub/drum bolt pattern. For the axle, that is the Dexter Trailer Axle Beam with E-Z Lube Spindles - 95" Long - 5,200 lbs # 6340624.
If you also happen to need the hubs/drums and the bearing kit as well, that is the Dexter Trailer Hub and Drum Assembly - 5,200-lb E-Z Lube Axles - 12" - 6 on 5-1/2 # 8-201-5UC3-EZ.
