I reached out to my contact at EcoHitch and she verified that the Hidden Trailer Hitch Receiver # 306-X7137 will fit with the full size spare tire beneath your 2008 Toyota Highlander. You will want to lower/remove the spare during the installation process to make things easier but once the hitch is in place you will be able to re-mount it.
