Mounting Holes Pattern of Round Snap Ring Swivel Jack

Question:
Im planning on removing the snap ring plate and pull pin to mount Item # TJB-2002S-B with bolts. Can you please provide me with the hole pattern so I know where to put the holes? Could you also provide me with the dimensions of the head of this jack? Thanks

The mounting holes are 3 inches apart on center for the Round Snap Ring Swivel Jack w/ Footplate - Weld On - Sidewind - 14-1/2" Travel - 2,000 lbs # TJB-2002S-B.
The head is 2-1/2 inches square for this jack as well.
