I purchased the 88221 air lift kit for an 89 suburban. It has 4 inch lift blocks in the rear. My question is do I need an angled 4 inch air bag spacer or a flat one?

You will need to get under your 1990 Chevrolet Suburban and look at the upper mounting bracket where the Air Lift LoadLifter 5000 Ultimate Air Helper Springs item # AL88221 attaches. If this bracket is angled, like the attached picture, you will need the Air Lift Angled Universal Air Spring Spacers item # AL49GV you referenced. If this bracket is flat then you will need the Air Lift Universal Air Spring Spacers item # AL52440.
