Question:

I just bought a 2019 Chevy 2500 HD with the puck system I have a Patriot 18K that I had in my previous track that was installed with the rails. Will the DM 6194 with my Patriot 18K DWRVK 3270

Unfortunately the B and W Patriot slider is the only hitch that can't be used with the Demco rail adapter part # DM6194 that you referenced. Your hitch looks to be in good condition so one solution might be to sell it used and then go with the Companion Slider # BWRVK3770 which fits your rail kit directly. The Companion hitch is essentially the same thing as the Patriot but fits to underbed rail kits like what your truck has so you'd get the same high quality hitch you have now but it would be a direct fit. While the adapter would have been a convenient solution I like to recommend direct hitch fit solutions whenever possible as there's a less likelihood for movement.
