I looked up the Take 3 auto transport trailer and it showed it has electric brakes. To convert your electric brakes to disc brakes you will need to use the disc brake conversion kits for 7,000-lb axles with the hub and rotor, hydraulic line kits for three axles, electric over hydraulic actuator, trailer breakaway kit, and a brake controller that is compatible with electric over hydraulic brakes.
The disc brake conversion kits will come in pairs, so you will need three sets. These kits are designed to fit 16 inch and larger wheels with an 8 on 6-1/2 inch bolt pattern. You will want to verify the part numbers of your inner and outer bearings to make sure you use the correct disc brake kit for your trailer. I am including a link to a product page that will list the disc brake kits I have available for 7,000-lb axles.
Once you have selected the disc brake setup you will need the following parts. Since you already are set up for using a brake controller, I would recommend the Titan Electric Over Hydraulic Actuator Kit, part # T4813102, to control the disc brakes with your brake controller. You will also need the Gel Cell Breakaway Kit for Titan BrakeRite Systems, part # T4822100, in case the trailer becomes detached from the tow vehicle.
Finally, you will need a brake line kit to complete the installation on the trailer. The Hydraulic Brake Line Kit for Tandem-Axle Trailers, part # 9579-2, would be correct for two axles with brakes. For the third axle you will need the Third Axle Brake Line Kit, part # 9579-01.
You will want to make sure your current brake controller can be used with electric over hydraulic brakes. If it is not compatible I would go with the Prodigy P3 Brake Controller, part # 90195, which is compatible with the parts recommended above. I am including a link to the instructions on the Prodigy brake controller.
