Question:
For 2019 Honda Pilot, there are hitches that fit with a spare tire, correct? It just can't be the large spare tire?

You are correct that your 2019 Honda Pilot will need to have a spare tire that is 18 inches or smaller in order for a trailer hitch to fit, so it needs to be smaller than a normal full-size spare tire. Should your Pilot have a smaller spare tire I highly recommend the etrailer # e98840 because it is a simple bolt-on installation that does not require any drilling into your Pilot's frame to install. I have added a link to a video of this hitch being installed on a Pilot and the accompanying photo shows it installed with a spare tire.
This Class III, 2" trailer hitch has a black powder coat finish which helps protect it from corrosion, a 6,000 lb towing capacity and 900 lb tongue weight capacity; it also comes with a lifetime warranty. I have added a link to a video of the etrailer # e98840 being installed for you to take a look at.
For wiring which will give you a 4-way connector and allow you to power your trailer's lights you need Wiring Harness # 118760. If your '19 Pilot has the factory tow package you can add a 7-way simply by adding T-One Vehicle Wiring Harness for Factory Tow Package - 7-Way Trailer Connector # 118286. I've added links to video reviews of this harness for you to check out as well.
I also recommend checking out Ball Mount Kit # 989900, which is for 2” hitches and may fulfill your towing needs in one tidy package; two ball mounts - one with a 3/4” rise or 2” drop and the other with a 2-3/4” rise or 4” drop - a 1-7/8” hitch ball, a 2” hitch ball, and a pin and clip.
You can use the link to the help article on ball mounts I've added to determine if this kit will give you the correct amount of drop or rise.
