There are 5 mounting holes how many of these do I need to use to mount this actuator

The Titan 8K Surge Coupler # T4747220 should be installed using all 3 of the smaller mounting holes or both of the larger holes (using hardware with dimensions that match) to secure it to the trailer's 3-inch-wide tongue.
