Demco 5th Wheel Rail Adapter # DM6099 is designed to fit the factory 5th wheel prep package that is in some 2013 to present Ram trucks. It would not fit your 2004 Ram 2500 since the factory prep package was not available for that model year.
Does your Ram have a 5th wheel installation kit already installed? What is the brand of your 5th wheel hitch? And does that hitch fit in above the bed rails or in an underbed system?
If you can give me a little more information I can recommend the parts you will need.
