Based on the bearing sizes # 25580 and # LM67048, there are two possible hubs that will fit.
The first has a wheel bolt pattern of 6 on 5-1/2 and fits axles rated for 5,200 lbs to 6,000 lbs. This hub can be purchased with the standard hub assembly # 8-213-5UC1 or the E-Z Lube axle hub assembly # 8-213-5UC1-EZ. Your bearing combination indicates that your axle capacity is 5,200 lbs.
The other hub option has a bolt pattern of 6 on 6 and fits axles rated for 6,000 lbs. This hub can be purchased with the agricultural hub assembly # 42660UC1. This hub assembly is primarily designed for agricultural applications.
