Based on the dimensions you listed the etrailer.com brake assembly kit part # AKEBRK-7-SA would be your best option. It is 12 inch by 2 inch and has the 5 bolt mounting pattern.
I attached a picture that shows the dimensions of the bolt pattern for you to check out as well.
