Greetings, I am looking to replace my kodiac 12inch disks. I am a little confused as to what bearings and pads go with the rotors. Can you help.

The Kodiak 12" Hub-and-Rotor Assembly # KHR12D that you referenced requires an inner bearing of # 25580 and an outer bearing of # 15123.

For a set of brake pads you would want the part # K225CP.

I recommend replacing your current surge brake coupler using either using the Demco Hydraulic Brake Actuator for Disc #DM8104311 or Drum #DM8102511 brakes. I prefer these couplers as they have a Zinc finish which best resists corrosion.

The bearing #LM67048 and #15123 that you referenced have the same inner diameter which means they fit the same spindle and the disc brake kit #K2HR526DKG would fit in place of what you have now.
