Why Do Yakima HD Bar EndCaps Keep Falling Off

Question:

After installing per instructions, these still just fall off, why? How do you prevent this?

If the EndCap for Yakima HD Crossbars # YA54FR keeps falling off then that leads me to believe you may not have the bolt and nut lined up correctly. If you check out the attached photo you'll see where the nut is supposed to sit on the endcap. With it in this location it will line up perfectly with the hole in the crossbar. Once the bolt is installed there should physically be no way for the endcap to fall off other than the bolt being extremely loose and falling out.
