The B and W hitch part # BWHDRH25122 that you referenced only fits the cab and chassis versions of your truck which have a narrower frame width than what you mentioned at 34 inches from outside to outside. So what you'd want is the part # BWHDRH25189 instead which is designed for the truck frame dimensions you have on your 2006 Chevy Silverado 3500HD.
