I have a 2018 GMC 1500 with a cab height topper. Will the Long Arm work to carry a canoe? I may need the extender to have enough height to clear the topper.

The Yakima LongArm Truck Bed Load Extender - 2" Hitches - Aluminum - 300 lbs # Y01149 that you were looking at is only suitable for truck bed-height loads. For a cab-height load you will want to go with the Yakima LongArm Bed and Roof Load Extender for 2" Hitches - Aluminum - 165 lbs # Y01149-1150.
I also recommend adding Crossbar Pad # Y07417 to prevent your canoe or other items from getting scratched. This measures 28" long and wraps around the Yakima Longarm crossbar. You could also add Yakima hitch lock # Y07235 to keep your valuable load extender safe and secured to your vehicle's hitch receiver. I attached demonstration videos for these products.
