On a 2020 sprinter 2500 144” factory step bumper, does the receiver mount on the outside of the frame rails, about 38- 3/4” outside of frame?

Yes the Draw Tite hitch part # 75601 does attach to the outsides of the frame rails of your 2020 Mercedes Benz Sprinter 2500. You can see this in the install instructions diagram that I attached to this page.
