I do have a great option for you, however the B&W Companion Slider 5th Wheel Hitch # BWRVK3400 likely won't work on a flat bed truck because the gooseneck hole would be too far recessed in the bed. It's designed to fit into the hitch ball hole for the B&W Turnoverball Gooseneck Hitch that fits standard truck beds, so it's just not sized appropriately to connect to a recessed hole like what you would find on most flat bed setups.
A much better option will be with the B&W Companion Flatbed # BWRVK3050 along with the corresponding Turnoverball Gooseneck for Flatbed Trucks # BWGNRK1500. This combo is specifically built to provide the appropriate attachment point on a flat bed and its low-profile design also helps with keeping the trailer level since a flat bed is going to be much higher than a standard pickup bed.
I have attached a short video review and a customer-submitted image of the hitch mounted that you can check out as well.
Just keep in mind that you will need to be careful when making sharp turns, as there isn't a slider version available of the Companion Flatbed. It might be a good idea to take the trailer up to an empty parking lot with a buddy so you can test your cab clearances when making tight maneuvers.
