We have a complete axle kit that meets your requirements, Dexter kit # 7461121-EB. This consists of a 6000-lb axle with a 95-inch hub face spacing (and 80-inch spring center spacing); please refer to the linked diagram for these dimensions.
The kit includes the axle beam and spindles, electric brake assemblies, hub/drums, bearings, races, seals, and mounting hardware. The hubs have the 6-on-5-1/2 bolt pattern and the spindles are the E-Z Lube type that allow simple lubrication. This kit is one axle so you will need two kits; pricing in noted on the kit's page.
On the linked page you'll see double eye leaf springs for 6K axles and these come in several lengths. Since worn springs tend to get slightly elongated from use you'll want to select new springs that are slightly shorter than your current ones. For instance, if your worn springs measure 26-1/2-inches from eye to eye then you'd want to use 26-inch spring # e94SR.
For a trailer brake controller that will install easily I suggest the Curt Echo # C51180 which is shown in the linked video. The Echo requires no splicing or wiring in the vehicle as long as it already has a working 7-way socket. If you have a compatible smartphone (with Apple 8.0 or newer or Android 4.4 or newer operating system) then the Echo offers by far the simplest installation available for a proportional brake controller. If you do not have a compatible phone or if you simply prefer a wired controller please tell me about your vehicle and I can make another recommendation.
