Yes, a brake controller will need to be hardwired into a 1991 Chevy C/K Series truck, though the installation is not something overly difficult.
To install the brake controller I recommend for your truck, the Tekonsha Prodigy P2 Trailer Brake Controller - 1 to 4 Axles - Proportional # 90885 you will need the Brake Control Plug-In Harness # 20127, which plugs into the Prodigy and has four wires, a black power wire, white ground wire, blue brake wire and red stop light switch wire.
If your '91 Chevy C/K does not have a 4-way you will need to start with that, either part # HM41115 if your truck is a Crew Cab or part # 118315 if your Chevy is not a Crew Cab. I've added links to some sample install videos for you to check out for these harnesses.
You will then need to add the Installation Kit for Trailer Brake Controller # ETBC7 which will give you a 7-way connector, circuit breakers and the rest of the hardware needed to install the Tekonsha Prodigy P2 # 90885. I have added a link to a video showing the # ETBC7 being installed on a 1994 Chevy C/K Series which will be the same as on your truck.
