Question:

will this hitch bolt up on 2003 F 550 cab and chassis with 34 1/4 frame w/ four rivets at the end of each rail instead of one in the center

The Draw-Tite Class IV Hitch, part # 41943 will fit the rivet configuration you mentioned (see figure # 3 in the instructions I have linked to). I checked with one of my contacts at Draw-Tite, who tells me that this hitch will work with the 34-1/4 frame measurement. That 1/4 inch is within acceptable tolerances, so the hitch should fit just fine for you.
