Hi I have a Titan XD diesel 2019 and looking at the Reese Gooseneck setup 20k 6.5 bed. Will this work? And do you recommend a 4-5inch extension for a 5th wheel RV

Yes, it will! The Reese Elite Series Pop-In Ball Kit item # RP30137 has been confirmed to fit the OEM gooseneck hitch that comes with the 2019 Nissan Titan XD.

Since you have the shorter 6-1/2' bed length, I do recommend using an offset gooseneck ball like the Curt Multi-Fit Ball Kit w/ 4" Offset item # C68ZV. This will push your hitch ball back 4" towards your tailgate to give you some extra turning clearance.

The Reese Goose Boxes like the part #RP85FR are only fifth wheel to gooseneck adapters that won't void the warranty of the RV manufacturers.
