While I can't say yes or no for if it will give you enough clearance I can give you the measurements so you can verify that you will have enough clearance for your Forest River Sierra 40002FB. First the 4" Offset Ball # BWGNXA4085 does give you 4" of additional clearance from your current hitch ball. The Gen-Y Hitch 5th Wheel to Gooseneck Pin Box # GY98GR is 16-3/16" from the center of the front mounting holes for the pin box to the ball center for the coupler. You will need to measure this on your Sierra to determine if the 5th wheel will be able to clear your flatbed corners when turning.

If it is not enough to get the clearance you need you might opt for the Convert-A-Ball Cushioned 5th-Wheel-to-Gooseneck Adapter # CAB-C5GX1216 instead of the Gen-Y. This will give you 7-1/2" of clearance.

Hey, were you able to measure the clearance on your Sierra 4002FB to see if the offset ball and Gen-Y hitch gave you the space you needed around the flatbed? Knowing what worked could help others trying to get similar clearance.
