Hello Larry. That is a very specific wheel and I will be honest with you, we do not have a wheel that will meet all of your requirements. This is mainly because the 4 on 4-3/4" bolt pattern is pretty uncommon these days. We offer only one wheel with this bolt pattern, the Steel Mod Silver Trailer Wheel # AM38NR. This wheel has a 15" diameter and is 5" wide. It does have the 5 on 4-3/4" bolt pattern but it does not have an offset. I know this is not exactly what you were looking for, but this will be our closest option.
