Hey Tony, thanks for writing in, and that's a really smart question. The answer is yes, the fresh water tank should be full when adjusting the 16,000 lb Equal-i-zer Weight Distribution System # EQ90-00-1600.
What you want to do is have the trailer loaded up just like it would be when travelling to ensure you get the proper adjustment and full benefits of the hitch.
As an aside, if you don't already have a hitch ball for the # EQ90-00-1600, the right one is the 2-5/16" Hitch Ball for Equal-i-zer Weight Distribution Systems # EQ54FR.
