What is the terminal and screw on the top fo? Possibly a ground?

Yes, the ring terminal you see attached to the housing of the Pollak Single-Pole, Round Pin Trailer Wiring Connector # PK11852 that you referenced is used for a ground circuit.
Check out the picture I attached that came from a customer who used this to install his winch. You can see the black (ground) wire run to the ring terminal.
