To replace your Polar Blue Horizon 21' Dometic Sunchaser awning you'll need the Solera XL 12V Power RV Awning - Blue Fade # LC75FR. At this length you'll want to also get a Solera Awning Cradle Support - White # LC289373 (black # LC289374) to help prevent the middle of the awning from sagging or wagging in the wind when on the road.
You'll also want to pick up some butyl tape # AL28FJ to seal up each mounting screw hole while installing the awning.
