Question:
I didnt receive instructions so can you tell me where the o-rings should be placed? Is it bolt, flat washer, o-ring, foot pad, and then second o-ring?

You are correct, the Replacement Footpad Kit # LC86UD is mounted with the bolt, flat washer, o-ring, footpad and finally the 2nd o-ring before threading into the base of the hydraulic jack.
