I wasn't able to match those numbers to any Westin part numbers, however:
If you have the older Westin Sportsman Grille Guard # 45-1640 and 40-164PK, then the replacement hardware is the Replacement Hardware Kit for Westin Sportsman Grille Guard # 40-164PK.
If you have the newer Westin Sportsman Grille Guard # 40-0125, then the replacement hardware is # 40-012PK.
