What is the replacement part? I think it may be LC1258802 from the picture I took of the gear box and motor, but I am not sure?

Based on the part number 1-065-041012-01 I was able to cross reference some prior requests for this and I determined you can use part # LC196308. Replacing the gearbox would be optional and for that you can use part # LG-179015. This is a landing gear motor setup though and not a slide out.
The motor you referenced is for a slide out and would not work for you.
