I want to make sure I get the right size rim. Is this a 5 x 5 or 5 x 4.5? Thank you

With you measuring 3 inches from the center of 2 holes right next to each other and then got 5 inches when you went center of one hole, skipped one, and then went to the back end of the next hole, you definitely have a 5 on 5 bolt pattern. That being said, the Taskmaster Provider ST205/75R15 Radial Trailer Tire w/ 15" Silver Mod Wheel # TA43ZR will be a good option as a replacement.
