I recently purchased a used trailer-form the looks of it a homemade one. The Current Hub is a 4x100 bolt pattern. I am wanting to switch it to a 6x5.5 bolt pattern. I am need some guidance on how to make sure I get the right hubs.

I can tell you how to determine the correct hub for your trailer, and you should be able to get one with a 6 on 5-1/2" bolt pattern, but it really depends on the spindle that is currently on the trailer. You do need to know the weight rating of your axle, as well as what bearings your current hub has.
To do this, you will need to take apart your current hub assembly and look at the bearings themselves; wipe away the grease and you should be able to see the part number stamped directly on them. If you are unable to find them, you need to use digital caliper and measure your spindle to the thousandths of an inch where the bearings and grease seal ride. You can use the photo I've attached to see exactly where you need to measure.
For example, if you have a 3,500 lb axle with inner bearing # L68149 (1.378" inner diameter) and outer bearing # L44649 (1.063" inner diameter) the hub with a 6 on 5-1/2" bolt pattern you will need is the Trailer Idler Hub Assembly for 3,500 lb Axles # AKIHUB-655-35-K.
