will the rear hatch foot-sensor still work once the hitch is installed?

You may have to kick to either side of the hitch opening, but yes you could still operate the hands-free liftgate of your 2018 Audi Q5 with the Draw Tite hitch part # 76183 installed. Due to how tightly this fits into the fascia opening for a hitch I'd be shocked if you even noticed a difference.
