Is there a different part number for right or left? Are the bolts and nuts included to mount the hub assembly to the axle?

The Trailer Hub and Drum Assembly # AKHD-545-35-EZ-K that you referenced can work on left or right sides of a trailer. This gets held on with a castle nut like part # 165686. Check out the video I attached on removing and reinstalling trailer hubs to see how it all comes apart and goes back together.
