Hello I have a 2017 Ford F-250 Referring to ITEM# C60692. What is the difference between a factory rail kit and no factory rail kit? Does the bed have to be removed to install the mounts that bolted to the truck frame? Thank you

The factory rail kit is also known as the factory prep package on your 2017 Ford F-250. The difference is that the prep package includes the 4 holes plus the gooseneck hitch ball hole in the center. You can see an example of this in the attached video.
If you do not have the factory rail kit (or factory prep package) then you will not have the 5 holes in your truck bed.
If you do have the factory rail kit then you can just purchase the Curt Ball and Safety Chain Loop Kit # C60692 that you referenced and you are good to start towing a gooseneck trailer!
If you don't already have a gooseneck hitch then I highly recommend the B and W Gooseneck Hitch, part # BWGNRK1116, which comes with safety chain loops and a hitch ball that stores upside down in the hitch when not in use. This is a very popular option that is made right here in the USA.
If you were to install a gooseneck hitch you would not have to remove your truck bed. I have attached an installation video that you can use as a reference.
