Question:

I am looking for something similar to this # RP30136 however the hole I have is 4 inches internally. is there a puck that fits this? I am unsure of what hitch or whatever was there before.

We don't have a puck cover like # RP30136 that has a 4 inch internal dimension. Most underbed gooseneck systems required a 4 inch hole cut for the unit, but that included the outside ring of the system and sometimes trim as well as the hole for the ball. It sounds like there was an underbed gooseneck system installed. For instance, the 2010 Ford F-250 and F-350 Super Duty B&W Turnoverball Underbed Gooseneck Trailer Hitch w/ Custom Installation Kit - 30,000 lbs, # BWGNRK1108, required a 4 inch hole. I've attached an installation of this system for you.

My recommendation is to install a replacement hitch or have a professional welder patch your bed.
