I went out to the warehouse to examine the CIPA Custom Towing Mirror, Driver Side, # CM11301, more closely. (The passenger side mirror is # CM11302.) The instructions do state that the towing mirror may mar the factory mirror finish. However, the wedge that holds the mirror in place comes with an adhesive backed felt liner to help protect the finish. The tightening knob on the back should be tight enough to secure the mirror but avoid over tightening as damage to the OEM mirror may occur.
The mirror is designed with aerodynamics in mind to prevent wind noise and vibration. You may still experience some noise and vibration, especially with a cross wind but the shape of the mirror is such as to make noise and vibrations as little as possible.
With the towing mirror attached, it will not interfere with the operation or adjustments of your existing OEM mirrors. The mirror may need periodic adjustments (tightening or repositioning) to keep it tight to the vehicle mirror so it does not fall off. I recommend give it a quick check before entering the vehicle as it only takes a few seconds to tighten it up if needed.
