I have a solution for you but the Buyers Products Hitch Plate with Receiver Tube - Chevy/GMC item # 3371809065 will not fit your 2002 GMC W4500 Forward. You will need to use one of our universal hitches like the Draw-Tite Ultra Frame Service Body Trailer Hitch Receiver item # 41990-07. This hitch is a weld on and you will be able to choose between two different mounting brackets depending on how much drop you need. I have attached a couple diagrams that will show you the dimensions of the two mounting brackets.
