How are trailer brake flanges measured - specifically this indicated 4 x 4inch flange? Distance between adjacent bolts or diagonally? Thanks! Evan

Both actually! They're measured center to center hole from left to right, top to bottom, and across diagonally like in the diagram below. The one in the picture below is the Brake Mounting Flange item # 4-35. What's also really important is the diameter of the center hole. This one's for a 2-3/8" round axle.
