We do have a very similar replacement coupler that is rated for 21,000 GVWR, and it is the Titan Bolt-On Coupler # T4855300.
The angle that your photo was taken I can not tell if it is a 2 or 3 hole adjustable coupler, but this Titan part is a 3 hole adjustable, 2-5/16 coupler that is rated for 21,000 pounds. This coupler fits 3-1/2 inch wide trailer tongue channels with 4 or 6 hole leveler channels.
This assembly includes the coupler that has a primed finish, 3 Grade 8 bolts that are 5/8 inch diameter, and 3 lock nuts.
