Is this item # LO34FR for 2019 Ram 1500 ADJUSTABLE? Up/Down and In/Out?

Yes, the Longview Custom Towing Mirrors - Slip On - Driver and Passenger Side # LO34FR have an adjustable mirror face. They can adjust up and down, side to side, or at an angle.
If by in/out you mean can the mirror retract into the housing or stick out further then no, that is not something the mirror face can do. If you mean can they extend out wider from the factory mirror and retract back in then the answer is also no. These slip on over the factory mirror and are in a fixed position with only the angle of the mirror being adjustable.
I have attached an installation video for your reference. If you jump to 3:20 in the video you will see how the mirror manually adjusts.
