I need this and the actual ball mount for it also, what are the part numbers?

The Equal-i-zer Weight Dist Shank # EQ90-02-4600 is designed to work with a weight distribution hitch head like # EQ90-02-1000. If this is what you are needing I would need to know what weight rating you need for your system.
If you are wanting to use it as an adjustable ball mount the Ball Mount Adapter # BA-1 does share the same 3/4" bolt holes spaced 1-1/4" apart. It will require a hitch ball that has a 1" shank.
1-7/8" Ball # 19260
2" Ball # 40038
2-5/16" Hitch Ball # C40005
Another option is the Curt Adjustable Clevis and Pintle Hook Combo # C45008. This comes with a 2" and 2-5/16" hitch ball mounted.
