I have a 1993 e350 rv at the rear frame it measures 42 inch frame from out side to outside ever time I try and look this up it gives me a hitch with a 38 inch wide that fits inside my frame and does not work and need class 4

Thanks for the frame dimensions, and while there are hitch options for your 1993 Ford E-350 RV, I checked with the hitch manufacturers and there are no custom fit options. So what that means is in order to add a hitch to your RV, you would need a universal weld-on hitch.
There are three different options, with each of them being Class V hitches with a 2 inch receiver, and fit frames that are between 18 inches and 44 inches wide, so they will fit your '93 E-350's 42 inch frame. The difference between them is the amount of drop they give from the frame, so you'll want to measure the amount of drop you need in order to select the right hitch. The three options are the following:
- 3" Drop: Class 5 Universal Weld-on Hitch and Brackets # 15901
- 9" Drop: Class 5 Universal Weld-on Hitch and Brackets # C47CR
- 18" Drop: Class 5 Universal Weld-on Hitch and Brackets # C99CR
