Do you have to remove entire grille to install base plate? Also, can I use my current Blue Ox tow bar with the base plate?

Yes, the bumper and front fascia of your 2018 GMC Sierra 1500 does need to remove to install the Demco Base Plate kit part # DM9519330. Then to use this base plate kit with your Blue Ox tow bar you'd just need the part # DM9523127 and you'd be set.
